Ionizing radiation and its sources are used every day in medical, industrial and governmental facilities around the world. Although some health risks from ionizing radiation exposures are widely recognized, the association of these exposures to specific diseases, especially various types of cancer, remains uncertain. Workers at U.S. Department of Energy (DOE) facilities have produced nuclear weapons, provided nuclear fuel materials for power reactors, and conducted a wide spectrum of research related to nuclear safety and other scientific issues. While completing this work, many of the employees have been exposed to ionizing radiation and other potentially hazardous materials. Since 1991, the National Institute for Occupational Safety and Health (NIOSH) has conducted analytical epidemiologic studies of workers at DOE nuclear facilities, through a Memorandum of Understanding between the DOE and the U.S. Department ofHealth and Human Services (DHHS). The agreement occurred in response to recommendations to the Secretary of Energy in 1989 by the independent Secretarial Panel for the Evaluation of Epidemiologic Research Activities (SPEERA). This technical report, entitled An Epidemiologic Study of Mortality and Radiation Risk of Cancer Among Workers at the Idaho National Engineering and Environmental Laboratory, a U.S. Department of Energy Facility, is one several products of the NIOSH Occupational Energy Research Program that are being published as a series. Most of these studies include detailed historical exposure assessments for radiation and other potentially hazardous agents so the health risks at different levels of exposure can be accurately estimated. Each of these studies contributes to the knowledge required to ensure that workers are adequately protected from chronic disease over their working lifetimes. The Idaho National Engineering and Environmental Laboratory (INEEL) is a large U.S. Department of Energy (DOE) facility near Idaho Falls, Idaho. Ever since the United States began producing and testing nuclear weapons during World War II, the effects of ionizing radiation on human health and the environment have been a serious public concern. The Worker and Public Health Activities Program was established more than 20 years ago to study the consequences of exposure to ionizing radiation and other hazardous materials from Department of Energy operations to workers and members of the surrounding communities. In 2005, the National Academies convened an expert committee to conduct a review of the Worker and Public Health Activities Program, which is operated by the Department of Health and Human Services (HHS) at Department of Energy (DOE) nuclear facilities under a Memorandum of Understanding (MOU) with DOE. Review of the Worker and Public Health Activities Program Administered by the Department of Energy and the Department of Health and Human Services concludes that the program has used sound research methods and generally has enhanced public understanding of the risks involved. However, the report recommends that more two-way communication between the agencies and workers and members of the public is needed. The report also explores the ways in which the agencies involved could develop a more coordinated, effective, and thorough evaluation of the public health concerns involved in cleanup and remediation activities at Department of Energy sites.

Current radiation protection standards are based upon the application of the linear no-threshold (LNT) assumption, which considers that even very low doses of ionizing radiation can cause cancer. The radiation hormesis hypothesis, by contrast, proposes that low-dose ionizing radiation is beneficial. In this book, the author examines all facets of radiation hormesis in detail, including the history of the concept and mechanisms, and presents comprehensive, up-to-date reviews for major cancer types. It is explained how low-dose radiation can in fact decrease all-cause and all-cancer mortality and help to control metastatic cancer. Attention is also drawn to biases in epidemiological research when using the LNT assumption. The author shows how proponents of the LNT assumption consistently reject, manipulate, and deliberately ignore an overwhelming abundance of published data and falsely claim that no reliable data are available at doses of less than 100 mSv.
