The events that took place on September 16, 2013 at the Washington Navy Yard were shocking and tragic. Twelve innocent lives were lost that day, along with several injured. On September 16, 2013, Aaron Alexis, a Navy contractor employee with a Secret security clearance, shot and killed 12 U.S. Navy civilian and contractor employees and wounded several others at the Washington Navy Yard. Alexis was also killed. This independent review, completed by the Secretary of Defense Independent Review of the Washington Navy Yard Shooting, examines the incident and suggests ways the Department may have missed clues or been more prepared to prevent the tragedy. The review was released by the Secretary of Defense in a news conference on March 18, 2014. This independent review was released on the same day as another, internal review was released. Like the independent, external review, the internal DOD review was completed in November 2013 but released on March 18, 2014.
