Alexis was also killed. This independent review, completed by the Secretary of Defense Independent Review of the Washington Navy Yard Shooting, examines the incident and suggests ways the Department may have missed clues or been more prepared to prevent the tragedy. The review was released by the Secretary of Defense in a news conference on March 18, 2014. This independent review was released on the same day as another, internal review was released. Like the independent, external review, the internal DOD review was completed in November 2013 but released on March 18, 2014.
