Since 1958 the Association for the Study of Internal Fixation (ASIF) has worked to establish the experimental and biomechanical prin ciples of the operative treatment of fractures. It has been proven that immediate, pain-free mobilization, made possible by stable internal fixation, is the key to restoring function in a fractured extremity. The same principles were applied in 1972173 by Roland Schmoker, who developed a universal plate and sophisticated instrument set designed initially for the treatment of comminuted fractures of the mandible. To extend the applications of his plate to reconstructions after ex tensive tumor resections, he supplemented the device with tem poromandibular joint prostheses and anchoring elements for the at tachment of dentures. He first tested the functional stability of these implants experimentally in sheep. Later he found that the minipig had a more human-like jaw shape and masticatory action, and so he tested the implants in 37 mini pigs that underwent extensive mandibular re sections without bone grafting, using sequential dye injections and postoperative weight gain to chart the progress of osseous regener ation. Seven other animals were not operated and served as controls.

The resection and reconstruction of craniofacial lesions are a challenge for the patient, surgeon and prosthetic dental providers. These procedures impact the patient's psychological, functional and esthetic concerns often affecting their ability to interact with other individuals. Dental providers must navigate constraints regarding occlusion, graft adaptation/stability, esthetic contours and duration of intervention. Novel computer-aided surgery (CAS) technology, bone grafting and prosthetically driven dental implant placement can overcome the limitation of traditional reconstructive approaches following mandibular resection procedures. The aim of this report is to present a multidisciplinary approach to treatment planning and surgical and prosthetic reconstruction of the lower jaw following resective surgery.A 23-year-old man was referred to the Oral and Maxillofacial Surgery clinic at the University of Connecticut, Farmington, CT, USA with a benign lesion (ameloblastoma) of the left mandible. Mandibular partial resection, including custom plates and cutting guides, and reconstruction of the defects with an iliac crest graft was planned and performed in two stages utilizing CAS technology. Following resection and mandibular reconstruction, prosthetic rehabilitation with implant supported Fixed Partial Denture (ISFPD) was planned, and dental implants were placed (NobelParallel CC).Mandibular reconstruction with the iliac crest graft was accomplished without complication. The cutting and assembling procedures were efficient and carried out uneventfully. Alignment and stable contact between bone segments was achieved with successful reconstruction of the mandibular shape. Using a prosthetically driven surgical guide, four implants were placed in the reconstructed area and primary stability was achieved. Oral function including chewing, swallowing and speaking was restored and the patient was satisfied with the esthetic outcome.Due to the many surgical and prosthetic considerations required for complex reconstruction cases, a functional and esthetic outcome can be achieved with close collaboration between the surgical and prosthetic dental providers both during comprehensive treatment planning and throughout the progression of treatment.
